New Reach Out and Read Raffle

Fairview Pediatrics participates in the program Reach Out and Read, a national organization that provides books and literacy awareness materials to medical practices.  By participating in this program, our office is able to provide a new, age-appropriate book to all of our patients ages 6 months to 5 years of age at the patient's well child visits.

In order to raise money to buy the books, our office holds several raffles and other types of fundraisers throughout the year.  We are currently raffling off a car organizer packed with auto safety supplies, see our Front Desk Staff for tickets!

Congratulations to Dena Vance, who won a Valentine's Day picnic basket in our latest raffle to benefit Reach Out and Read!  A big Thank You to all who participated in our fundraiser!

School, Sports, and Camp Forms

Due to the extremely high volume of school, camp, and sports forms received at this time of year, please expect a 5-7 business day turnaround time for completion of your forms. 

Some important things to keep in mind for form completion:

-We complete forms on a first come, first served basis during regular business hours.  We do not complete forms during our evening hours or on weekends.

-If your child has not had a physical exam in the last 12 months, we will not be able to complete your child's form until he or she is seen for a physical exam.  When you drop off a form, we will check the date of your child's last physical exam. 

-Please complete all parent portions of the form before you drop it off to our office.  This is especially important for MIAA High School sports forms. 

-For medication forms, please write the name, type, and strength of the medication to be dispensed to your child on the form.  For example:  "Ibuprofen 200mg tabs", not just "Ibuprofen."  The medication form, by law, has to state exactly what medication is being dispensed, and your child's school nurse can only dispense a medication exactly as described on the form.

-If your child comes in for a physical exam and you have a form (or forms) that you need completed, please give them to the medical assistant right away.  The medical assitant completes the majority of the form, not the provider.  If you have more than one form needing completion, we may not be able to complete all forms at the visit, depending on the length and complexity of the forms. 

-Your patience and cooperation are very much appreciated.      

Electronic Health Records Launch

Our office is currently preparing to launch our Electronic Health Records system.  We are very excited about these changes, and look forward to all the benefits EHR has to offer.  We apologize in advance if the office doesn't run as fluidly as you have grown accustomed to during our transition.  Our hope is that we will be back to normal and performing even better for you, our valued patients, as soon as possible.   At your visits you will notice that the Physicians and office staff will be utilizing small computer devices that will allow us to access your health information.  In the future we hope to see smooth flow in all aspects of your care, from telephone calls to office visits.  Please feel free to ask questions about how our new system will make life easier for you as our patient.  Again we appreciate your understanding and support during this substantial change and hope that you know how much we value your family as part of the Fairview Pediatrics family!

Our new EHR system requires that we obtain accurate demographic information. This will allow for effective insurance billing and allow us to ensure safety in medication prescribing, as well as many other things. We ask that you take the time to fill out this form prior to your visit to ensure that your visit is not delayed in any way. We appreciate your effort in being proactive in providing our office with your information, and encourage that you call our office with questions about the form.
