It's Not Too Late to Get Your Flu Vaccine

Flu Illness Activity is increasing in our area recently—it's not too late to get your flu vaccine and protect yourself!

Although no further Flu Clinics are currently scheduled, we still have plenty of vaccine available! If you want your child to receive the Flu Vaccine, please call our office at (413) 593-1333 to schedule an appointment.

New Office Hours

As of January 1, 2012, our office hours will be changing to the following:

  • Wednesday 8:00am-5:00pm
  • Thursday 8:00am-7:00pm
  • Friday 8:00am-5:00pm
  • Saturday 9:00am-12:00pm  (Our staff starts taking phone calls at 8:00am, hours can vary depending on volume of calls received)
  • Sunday  Available as per doctor request

Electronic Health Records Launch

Our office is currently preparing to launch our Electronic Health Records system.  We are very excited about these changes, and look forward to all the benefits EHR has to offer.  We apologize in advance if the office doesn't run as fluidly as you have grown accustomed to during our transition.  Our hope is that we will be back to normal and performing even better for you, our valued patients, as soon as possible.   At your visits you will notice that the Physicians and office staff will be utilizing small computer devices that will allow us to access your health information.  In the future we hope to see smooth flow in all aspects of your care, from telephone calls to office visits.  Please feel free to ask questions about how our new system will make life easier for you as our patient.  Again we appreciate your understanding and support during this substantial change and hope that you know how much we value your family as part of the Fairview Pediatrics family!

School and Sports Physicals

Physical Exams and Forms

We generally book physical exam appointments 1 to 3 months in advance. Summer physical appointments fill up quickly. Calling at least one month in advance helps us to have more appointment options available for you when you call. If your child is going to be playing sports, or entering Kindergarten, 4th grade, or 7th grade in school, a recent (within the last 12 months) physical is needed. Appointment spots fill up fast, so please plan accordingly.

The volume of forms that we receive for school and sports can vary, so if you need a form filled out for your child, we appreciate you planning in advance. The turnaround time for form completion is usually 1-2 days, but may become longer depending on the volume of forms we are receiving.

Before dropping off a form to our office, please make sure that your child has had a physical exam here within the last 12 months. If it has been longer than 12 months since your child's last physical, we will need to schedule a physical exam for your child in order to complete your form. Also, please review your school and sports forms and complete the parent portions of the forms before dropping it off at our office for completion. This is especially important for MIAA sports forms, which can not be completed until the athlete questionnaire is completed on the front of the form.

Please also keep in mind that we cannot fax any forms. We can mail forms back to you if you provide our office with a self-addressed, stamped envelope.
